*The Forms below will be completed ONLINE when you create or renew your USAV Membership on Sportsengine. Please make sure you do them when you are in the process of taking care of your membership.

Birth Certificate (You will have to submit this every year, save it on your computer or phone)

All athletes must show proof of current USAV membership when they arrive to open house, tryouts or they can send to Matt (only need to do it once at whichever one comes first for you).
